You may be eligible for “Special Protection” from termination of service. Your service cannot be shut off if you meet certain eligibility requirements. Information in the form includes:

• Financial Hardship Certification Request Form
• Age 65 and Older Protection Certification Form
• Third Party Notification Service Request Form
• Doctor Certification Form
• Supplemental Security Income Certification Form (required annually)
• Information on our Arrearage Management Program (AMP)
